November 2008 Flu Clinic Schedule

While supplies last, our Flu Clinics are open to both patients and non-patients.  For our patients, we will bill your insurance company, please be sure we have a copy of your current insurance card on file.  All non-patients will be charged $25 payable at the time of service with cash, check, MasterCard or Visa. 

Dublin Location:

Tuesday, November 11, 2008:  9:00 am - Noon

Friday, November 21, 2008:  2:00 pm - 4:00 pm

New Albany Location:

Thursday, November 6, 2008:  3:00 pm - 5:00 pm

Friday, November 14, 2008:  9:00 am - Noon
